三維彩色室壁運(yùn)動技術(shù)的臨床研究

摘 要 為了探討彩色室壁運(yùn)動技術(shù)(CK)三維重建的方法學(xué)并檢驗(yàn)這一新技術(shù)定量室壁運(yùn)動的
可靠性, 在25例患者中進(jìn)行了多平面經(jīng)食管左室兩維超聲和CK 技術(shù)的檢查, 利用自制的計算機(jī)軟件對
左室腔容積和CK 圖像進(jìn)行三維重建, 結(jié)果顯示: 左室三維CK 技術(shù)可立體顯示左室的室壁運(yùn)動幅度,左室三維重建法測量的心搏量與左室CK 體積高度相關(guān)( r= 0. 92, P 值< 0. 001) , 表明三維CK 技術(shù)是
定量分析室壁運(yùn)動幅度的可靠方法, 這一技術(shù)為估測心肌缺血范圍提供了新的途徑。

chocardiograph ic and CK studies w ere perfo rmed in 25 cases w ith hear t diseases using a mult ip lane

ray w as ro tated every10° th rough a 0°～ 180° arc to reco rd the lef t vent r icular two- dimensional and CK
images w h ich w ere reconst ructed to obtain 3- D images of the lef t vent r icular cavity and CK vo lume by
means of sof t- w are p rogram. The lef t vent r icular end- diasto lic (EDV ) , end- systo lic (ESV ) and st roke
vo lume (SV )w ere calculated f rom the lef t vent r icular 3- D images, and the vo lume at the outer (Vo) and
inner (V i) bo rders of the CK band and the vo lume occup ied by the CK band (V c)w ere calculated f rom
the lef t vent r icular 3- D CK images . The result show ed that lef t vent r icular 3- D CK images could dis2
p lay the ent ire lef t vent r icular w allmo t ion in a th ree- dimensional fo rmat and there w ere h igh co r rela2
t ions betw een EDV and Vo ( r= 0194) , betw een ESV andV i( r= 0194) and betw een SV andV c ( r= 0192).
It is concluded that CK is a reliable technique fo r quant itat ive assessment ofw allmo t ion and lef t vent r ic2
ular 3- D CK p rovides a p rom ising app roach to disp laying the extent of myocardial ischem ia .

彩色室壁運(yùn)動顯示技術(shù) (co lo r k inesis,CK)是一種基于聲學(xué)定量原理, 以彩色編碼實(shí)
時和連續(xù)顯示心動周期中室壁運(yùn)動幅度的一種
新型超聲技術(shù)[ 1 ]。然而, 這一技術(shù)僅能提供兩維
切面中室壁運(yùn)動的圖像, 左室室壁運(yùn)動的三維
形態(tài)在國內(nèi)外文獻(xiàn)中尚未見報道, 能否應(yīng)用 三
維CK 技術(shù)定量分析室壁運(yùn)動尚不明了。因此,本文的研究目的是: ①探討CK 圖像三維重建
的方法學(xué); ②檢驗(yàn)三維CK 定量分析室壁運(yùn)動
的可靠性。
資料和方法
一、研究對象、包括25例接受多平面經(jīng)食管超聲心動圖檢
查的連續(xù)患者, 男2 0例, 女5例, 年齡3 0～

塞14例(前間壁2例, 前壁3例, 側(cè)壁1例, 下壁5
例, 前壁+ 下壁2例, 前間壁+ 下壁1例) , 勞力型
心絞痛2例, 高血壓性心臟病4例, 二葉式主動脈
瓣1例, 主動脈瓣脫垂1例, 先心病房間隔缺損1
例, 無異常發(fā)現(xiàn)者2例。所有患者均為竇性心律,心功能é ～ ? 級。
二、研究方法
1. 使 用 儀 器: 采 用 美 國 惠 普 公 司 產(chǎn)
SONO S- 2500型相控陣超聲顯像儀, 多平面經(jīng)
食管超聲探頭頻率5. 0? 3. 75MHz, 其技術(shù)參數(shù)
已有另文報告[ 2 ]。該機(jī)配備有彩色室壁運(yùn)動顯
示技術(shù)(CK) , 該技術(shù)基于聲學(xué)定量(acou st ic
quan t if icat ion,AQ )原理, 自動識別和實(shí)時跟蹤
心內(nèi)膜的組織- 血液界面, 并根據(jù)心電信號將
收縮期中每兩幀圖像間的界面位移以不同的色
彩加以表示, 收縮早期第一幀圖像的界面位移
顯示為橙色, 之后每幅圖像的界面位移以橙色
→黃色→綠色→青色→藍(lán)色的次序進(jìn)行色階變
換, 收縮期界面的外向位移即矛盾運(yùn)動則以紅
色加以顯示, 至收縮末期所有彩階均疊加顯示
于最后一幀圖像中。
2. 多平面經(jīng)食管超聲心動圖檢查: 采用我
們以往所報告的方法進(jìn)行術(shù)前準(zhǔn)備和食管插
管[ 2 ]
, 待探頭插至食管下段后, 仔細(xì)調(diào)整探頭的
深度, 使相控陣裝置在0°～ 180°的旋轉(zhuǎn)過程中
均能清楚顯示左室腔輪廓, 然后固定探頭位置,于患者平靜呼吸狀態(tài)下, 自0°～ 180°依次旋轉(zhuǎn)
相控陣裝置, 每隔10°記錄至少3個心動周期的
左室切面圖像, 并與心電圖同步錄像供嗣后分
析 ......
