冠脈內(nèi)超聲評(píng)價(jià)冠心病介入療效的研究

【摘要】 目的 應(yīng)用冠脈內(nèi)超聲( ICUS)評(píng)價(jià)冠心病患者經(jīng)皮冠狀動(dòng)脈球囊成形術(shù)(PTCA)及冠脈內(nèi)支架植
入術(shù)的療效。方法 14 例患者在冠脈介入性診斷和治療前后行 ICUS檢查。共檢查 14 支靶血管 ,包括左前降
支 9 支 ,左旋支 1 支 ,右冠脈 4 支。結(jié)果 ①6 例 PTCA 術(shù)后即刻 ICUS檢查發(fā)現(xiàn) 5 例存在血管夾層和斑塊破
裂 ,占 83. 3 % ,5 處夾層部位均為鈣化或混合斑塊。其中 2 例尚可見小內(nèi)膜撕裂片在管腔內(nèi)隨血流飄動(dòng) ,而冠狀
動(dòng)脈造影(CAG)僅發(fā)現(xiàn) 1 例(16. 7 %)有明顯夾層( P = 0. 038) ; ②7 例支架植入術(shù)后 CAG結(jié)果均滿意 ,但符合
ICUS最佳支架植入標(biāo)準(zhǔn)者僅 2 例(28. 6 %) ( P = 0. 013) ,另 5 例靶病變?yōu)殁}化、混合斑塊者 ,支架膨展差或存在
游離腔隙。其中 4 例在支架邊緣處存在內(nèi)膜撕裂 ,2 例在術(shù)后 3 個(gè)月和 6 個(gè)月分別發(fā)生支架及支架近端再狹窄;
③19 例次超聲檢查中 ,發(fā)生一過性靶血管痙攣 1 例。結(jié)論 ICUS能準(zhǔn)確判斷 PTCA 術(shù)后并發(fā)癥 ,對(duì)支架成功
植入有重要指導(dǎo)價(jià)值 ,可作為一種與 CAG互為補(bǔ)充的診斷方法。

【Abstract】 Objective To evaluate the coronary artery response to percutaneous t ransluminal coronary
angioplasty (PTCA) and int racoronary stenting by int racoronary ult rasound( ICUS) . Methods ICUS was performed
in 14 patients before and after coronary intervention procedures. Target vessels imaged by ICUS included nine LAD ,one
LCX and four RCA. Re sults ① Six patients underwent ICUS imaging after PTCA. In five cases ( 83. 3 %) , the
dissection or plaque f racture at the site of angioplasty was observed ,and small intimal fla ped into the lumen in two of
them. The plaque composition of these target lesions was calcified or mixed plaque. However ,only one of them (16. 7 %)
had been diagnosed angiographically ( P = 0. 038) . ② ICUS was performed in seven patients after int raconorary
stenting. Coronary angiograms were considered as an acceptable results in all cases with stenting ,but only two of them
achieved the ICUS criteria for optimal stent deployment ,five cases showed incomplete stent apposition or inadequate
stent expansion( P = 0. 013) ,vessel intimal tearing were found at the edge of stents in four cases ,two of them revealed
restenosis at the margin of the stent or in 2stent during three and six months after stenting ,respectively. ③Only one case
had t ransient vessel s pasm during the procedures ,but recovered after catheter pullback and int racoronary nit roglycerin.
Conclusions ICUS can assess the coronary angioplasty results accurately and has an important role in guiding the
optimal stent placement .

冠脈內(nèi)超聲( ICUS)是近年來發(fā)展起來的一項(xiàng)新
的侵入性影像檢查技術(shù) ,國(guó)外已常規(guī)應(yīng)用于冠心病的
介入性診斷治療中。本研究通過與冠狀動(dòng)脈造影
(CAG)對(duì)比 ,旨在評(píng)價(jià) ICUS 對(duì)冠心病患者經(jīng)皮冠狀
動(dòng)脈球囊成形術(shù)(PTCA)及支架植入術(shù)的介入療效 ,探
討其臨床實(shí)用價(jià)值。
資 料 與 方 法
一、臨床資料
自1997 年 12 月至 1998 年 4 月對(duì)我院 14 例冠脈
介入治療患者進(jìn)行了 19 例次的 ICUS 檢查。男性 10
例 ,女性 4 例 ,年齡 50～77 歲 ,平均(68 ±6)歲。其中
穩(wěn)定性心絞痛 7 例 ,不穩(wěn)定心絞痛 5 例 ,胸痛原因待查
2 例。單支血管病變4 例 ,兩支6 例 ,三支2 例 ,正常血
管2 例。C 型靶病變 5 例 ,B 型 7 例。10 例行 PTCA
術(shù) ,8 例植入支架。支架類型包括 NIR支架 6 枚 , IRIS
Ⅱ支架 1 枚 ,SAINT2COME支架 1 枚。De Novo 支架
植入 6 例 ,Suboptimal 植入 2 例。球囊擴(kuò)張壓力 13～
16 大氣壓(1 大氣壓 = 101. 3 kPa) ,擴(kuò)張時(shí)間10～30 s。
術(shù)后 TIMI血流 Ⅲ級(jí) ,未見并發(fā)癥。
二、儀器與方法
使用CIVS INSIGH Ⅲ型血管內(nèi)超聲顯像儀 ,機(jī)械
旋轉(zhuǎn)式超聲導(dǎo)管 ,探頭頻率 30 MHz ,外徑 2. 9 F。首
先以J udkin 法完成冠狀動(dòng)脈造影(CAG) 。根據(jù) CAG
