POEMS綜合征的臨床表現(xiàn)及診斷

摘要:目的 分析 POEMS綜合征的臨床表現(xiàn)及診斷標(biāo)準(zhǔn)。方法 回顧性分析 36例 POEMS綜合征患者的臨床特點(diǎn) ,并與國(guó)內(nèi)、 外文獻(xiàn)進(jìn)行比較。結(jié)果 POEMS綜合征患者除了具有多發(fā)性神經(jīng)病 (100% )、 器官腫大 (92% )、 內(nèi)分泌
病 (86% )、 單克隆漿細(xì)胞增生 ( 100% )和皮膚病變 ( 86% )的典型臨床特征外 ,還常常表現(xiàn)為血管外容量過(guò)負(fù)荷
(97% )、 視乳頭水腫 (57% )和骨病變 (25% )。另外 , POEMS綜合征患者還常常伴發(fā)有 Castleman病 (25% )。結(jié)論
POEMS綜合征的診斷除應(yīng)將多發(fā)性神經(jīng)病和克隆性漿細(xì)胞疾病作為主要診斷標(biāo)準(zhǔn)外 ,還應(yīng)該包括硬化性骨病變、Castleman病、 視乳頭水腫、 外周性水腫和漿膜腔積液、 皮膚病變、 臟器腫大及內(nèi)分泌病變?cè)趦?nèi)的次要診斷標(biāo)準(zhǔn)。

Abstract: Objective To analyze the clinicalmanifestati ons and the criteria for the diagnosis of POEMS syndrome .
Methods The clinical characteristics of 36 cases of POEMS syndr ome were retros pectively reviewed and compared
with the cases reported in literature . Results I n additi on t o the typ ical characteristics of polyneuropathy (100%) ,organomegaly (92% ) , endocrinopathy (86% ) , monocl onal p las map r oliferative dis order (100% ) and skin chan2
ges (86% ) , the patients of POEMS syndrome als o have other i mportant features including extravascular volume o2
verl oad (97% ) , pap illedema (57% ) and bone lesi ons (25%) . Further more, 25% of POEMS syndr ome patients
have co2 existent Castleman disease . Conclusion To make the diagnosis of POEMS syndr ome, both maj or and mi2
nor criteria are required . The for mer includes polyneuropathy and monocl onal p las map roliferative dis order and the
latter includes osteoscler otic bone lesi ons, Castleman disease, pap illedema, organomegaly, edema or serous cavity
effusi on, endocrinopathy and skin changes .

臨床特征 ,如杵狀指、 皮膚色素沉著、 淋巴結(jié)腫大、 踝

將這組疾病綜合征
正式命名為 POEMS綜合征 ,分別代表多發(fā)性神經(jīng)
病 ( P)、 器官腫大 (O)、 內(nèi)分泌病 ( E)、 M蛋白 (M)和
皮膚病變 ( S) ,并被廣泛應(yīng)用至今。2003年 , Dis pe2
nzieri等[ 3～4 ]
對(duì)該中心收治的 99例 POEMS患者進(jìn)
