MRU在上尿路梗阻病因診斷中的應(yīng)用研究

【摘要】 目的 探討磁共振尿路水成像(MRU)對診斷上尿路梗阻病變的價值。方法 78例上尿路梗阻患者用重T 2 WI快速自旋回波(FSE)序列做MRU，采集資源影像后做最大信號強(qiáng)度投影(MIP)處理，其中23例同時應(yīng)用SSFSE序列及厚層投射技術(shù)直接成像。結(jié)果 78例患者經(jīng)過MRU檢查均滿意地顯示了梗阻部位及尿路擴(kuò)張程度，定位診斷準(zhǔn)確率為100%，定性診斷準(zhǔn)確率為88.5%。結(jié)論 MRU能準(zhǔn)確地顯示尿路梗阻的部位和程度，具有無創(chuàng)傷、無痛苦、無輻射、不需造影劑及能多平面成像等特點，比臨床上常用的B超、靜脈尿路造影、逆行腎盂造影等檢查具有一定的優(yōu)越性。特別適用于靜脈尿路造影(IVU)有禁忌證和腎功能喪失患者。關(guān)鍵詞 磁共振尿路水成像 尿路梗阻 診斷

【Abstract】 Objective To evaluate the diagnostic value of magnetic resonance urography(MRU)in patients with upper urinary tract obstruction.Methods 78patients with upper urinary tract obstruction were examined with heavy T 2 weighted fast spin echo pulse sequence to acquire image and postprocedure processing was performed with maxmium-inˉtensity-projection(MIP).Results MRU examination succeeded in all78patients and the sites of urinary tract obstrucˉtion were distinctly shown.The accuracy of MRU in the detection of the degree and level of urinary tract obstruction was100%and the accuracy for evaluating the causes of obstruction was88.4%.Conclusion MRU is noninvasive and free from ionzing radiation or iodinated contrast medium.It had more superior diagnostic value than ultrasound B ......
