聯(lián)合測定心型脂肪酸結(jié)合蛋白、肌鈣蛋白T診斷早期急性心肌損傷

深圳市第七人民醫(yī)院 汪潤 余楊 朱紅秋 朱平安【摘要】目的：采用聯(lián)合測定心型脂肪酸結(jié)合蛋白（heart fatty acid binding protein， h-FABP）、肌鈣蛋白T（cTnT）探討盡可能早期診斷急性心肌損傷的方法。方法：對76例急性冠脈綜合征（ACS）患者，自胸痛發(fā)生后一小時(shí)開始每小時(shí)連續(xù)監(jiān)測心型脂肪酸結(jié)合蛋白、肌鈣蛋白T至胸痛發(fā)生后6小時(shí)，共六次。以h-FABP>15μg/L、cTnT>0.1μg/L作為陽性標(biāo)準(zhǔn)，分析h-FABP或/和cTnT陽性結(jié)果對早期診斷ACS的意義。結(jié)果：在早期急性心肌損傷中，h-FABP陽性結(jié)果出現(xiàn)的時(shí)間早于cTnT，二者對比有統(tǒng)計(jì)學(xué)意義（P<0.05）。在cTnT升高病人中， h-FABP升高率為98.7%，二者對比無統(tǒng)計(jì)學(xué)差異（P>0.05）。結(jié)論：在診斷早期急性心肌損傷時(shí) ，h-FABP出現(xiàn)陽性結(jié)果時(shí)間早于cTnT，而聯(lián)合測定h-FABP、cTnT對提高ACS患者早期確診率，指導(dǎo)臨床急救具有重要的價(jià)值。

Combination determination for heart fatty acid binding protein( h-FABP) and cardiac Troponin T (cTnT) in early diagnosing acute myocardial injury
Summary
Objective To estimate a way for quickly diagnosing acute myocardial injury by measuring heart fatty acid binding protein( h-FABP) together cardiac Troponin T. Methods 76 patients with acute conoary syndrome (ACS) were studied. H-FABP and cTnT was measured in these patients from one hour after chest pain, once an hour, together six times. h-FABP>15μg/L or cTnT>0.1μg/L was positive. We estimated the clinical value of h-FABP or cTnT positive content in quickly diagnosing acute myocardial injury. Results h-FABP positive content occurred earlier than cTnT in acute myocardial infarction. The difference is significant. The h-FABP positive rate is 98.7% among the patients with cTnT content >0.1μg/L. The difference of positive rate is no significant. Conclusion The h-FABP positive content occurred earlier than cTnT in early diagnosing acute myocardial injury. Combination determination for h-FABP and cTnT is useful for early diagnosis of acute myocardial injury and important to direct clinical emergency. ......
