腎上腺髓質(zhì)脂肪瘤16例超聲與CT圖像對比

【摘要】 目的 探討腎上腺髓質(zhì)脂肪瘤的超聲及CT診斷價值。 方法 回顧性分析16例腎上腺髓質(zhì)脂肪瘤患者的超聲、CT及臨床資料，分析其聲像圖特點及超聲、CT診斷的準(zhǔn)確性。結(jié)果 17個腎上腺髓質(zhì)脂肪瘤病灶中，右側(cè)10個，左側(cè)5個，雙側(cè)1例。超聲表現(xiàn)為均質(zhì)型強回聲11個，強回聲區(qū)內(nèi)分布低回聲4個，囊實混合性回聲1個，1個漏診，彩色多普勒超聲檢查上述病灶內(nèi)均未見明確血流信號。超聲和增強CT診斷為腎上腺良性占位的敏感性均為94.1%，診斷為腎上腺髓質(zhì)脂肪瘤的敏感性分別為23.5%、82.4%，增強CT診斷腎上腺髓質(zhì)脂肪瘤敏感性明顯高于超聲(P<0.05)。結(jié)論 超聲可作為篩選腎上腺髓質(zhì)脂肪瘤的首選檢查和隨訪觀察方法，而增強CT對腎上腺髓質(zhì)脂肪瘤的定性診斷十分有價值。【關(guān)鍵詞】 腎上腺髓質(zhì)脂肪瘤；超聲；診斷；CT

【Abstract】 Objective To evaluate the characteristic features and diagnostic value of ultrasonography and CT on adrenal myelolipoma. Methods The clinical and ultrasound data of 16patients were analyzed retrospectively. Results In 16 patients, 17myelolipoma were found, of which 10 were in right adrenal gland, 5 in left and one patient was bilateral. Internal echoes: 11 cases were homogeneous with high echoes, 4 cases were high echoes with low echoes, 1 cases was high echo with echoless, 1 cases was missed diagnosis. Blood signal were not found in all caseson on color pepoler US. sensitivity of diagnostic ultrasonography and CT on benign tumor of adrenal gland were 94.1%, Sensitivity of diagnostic ultrasonography and CT on adrenal myelolipoma were 23.5%、82.4% respectively, CT obvious surpass ultrasonography in diagnosis of adrenal myelolipoma(P<0.05).
