烤瓷熔附金屬修復體在前牙美容中的應用及對患者牙齒美觀評分的影響(1)
[摘要]目的：探討烤瓷熔附金屬修復體在前牙美容中的應用及對患者牙齒美觀評分的影響。方法：選擇2015年1月-2017年2月筆者科室進行前牙美容修復治療的164例患者，根據(jù)修復體不同分為對照組(n=82例)和觀察組(n=82例)。對照組采用玻璃陶瓷全瓷冠修復，觀察組采用烤瓷熔附金屬修復體，兩組均連續(xù)進行12個月修復，修復完畢后對患者進行療效評估。采用美國公共健康協(xié)會(USPHS)量表從固位、顏色匹配、邊緣著色、繼發(fā)齲、解剖形態(tài)、表面光滑度、敏感度對兩組牙齒美觀進行評估；記錄并統(tǒng)計兩組修復后基牙疼痛、牙過敏疼痛、牙齦炎及食物嵌塞發(fā)生率；采用咬合力測定儀測定患者治療前、治療后12個月咬合力；采用稱重法對兩組治療前、治療后12個月患者咀嚼效率進行評估，比較兩組修復效果及對患者牙齒美觀、咀嚼功能的影響。結(jié)果：觀察組烤瓷熔附金屬修復體修復后12個月固位、顏色匹配、邊緣著色、繼發(fā)齲、解剖形態(tài)、表面光滑度、敏感度評分，均高于對照組(P<0.05)；觀察組修復后12個月基牙疼痛、牙過敏疼痛、牙齦炎及食物嵌塞發(fā)生率與對照組比較，均低于對照組(P<0.05)；治療后，觀察組咬合力與咀嚼效率與對照組比較，均高于對照組(P<0.05)。結(jié)論：將烤瓷熔附金屬修復體用于前牙美容修復中有助于提高牙齒美觀評分，并發(fā)癥發(fā)生率較低，且未對患者咀嚼功能產(chǎn)生影響，值得臨床推廣應用。

Methods From January 2015 to February 2017， 164 patients with anterior teeth were selected and divided into control group (n=82 cases) and observation group (n=82 cases). The control group was repaired with a glass-ceramic all-ceramic crown， and the observation group was treated with a porcelain-fused metal restoration. The two groups were continuously repaired for 12 months. After the repair， the patient's effect was evaluated. The US Public Health Association (USPHS) scale was used to evaluate the aesthetics of the two groups from retention， color matching， edge coloring， secondary caries， anatomy， surface smoothness， and sensitivity； Pain， tooth allergy pain， gingivitis and food impaction rate； occlusion force was measured by occlusion force tester before treatment and 12 months after treatment； patients were evaluated by weighing method before treatment and 12 months after treatment The chewing efficiency was evaluated to compare the effects of the two groups and the effects on the patient's teeth and chewing function. Results The 12-month retention， color matching， edge coloration， secondary caries， anatomical morphology， surface smoothness and sensitivity score of the porcelain-fused metal restorations in the observation group were higher than those in the control group (P<0.05). The incidence of abutment pain， tooth allergy pain， tendonitis and food impaction was lower in the observation group than in the control group (P<0.05). The occlusal force and chewing efficiency of the observation group were higher than those in the control group (P<0.05). Conclusion The use of porcelain fused metal restorations for cosmetic restoration of anterior teeth can improve the aesthetic score of teeth， and the incidence of complications is low.
