新生兒聽力篩查在新生兒聽力疾病診斷中的臨床意義(1)
[摘要] 目的 探討新生兒快速腦干誘發(fā)電位聽力篩查結(jié)果分析及臨床意義。 方法 收集2014年1月~2015年1月本院3000例聽力篩查新生兒臨床資料進(jìn)行分析。應(yīng)用快速腦干誘發(fā)電位聽力篩查儀對3000例新生兒做快速、無創(chuàng)的腦干聽覺誘發(fā)電位檢測,了解新生兒聽力障礙的發(fā)病情況。 結(jié)果 本院活產(chǎn)新生兒為3000人,初篩未通過472人,初篩陽性率為15.7%(472/3000),出生后42 d,464例復(fù)篩,442例通過,復(fù)篩通過率為95.3%,未通過22例,未通過率為4.7%。20例轉(zhuǎn)到本市聽力篩查中心,最終確診為聽力障礙(雙耳感音性耳聾)7例,系中度聽力損失,給予干預(yù)治療,占篩查總數(shù)的0.23%,8例隨訪至1歲,3例于4月時(shí)、4例于6月時(shí)經(jīng)聽性腦干反應(yīng)檢查均正常。不同分娩方式、胎齡、出生體重等指標(biāo)與聽力損傷密切相關(guān)。 結(jié)論 通過腦干誘發(fā)電位聽力篩查,新生兒聽力損失可早發(fā)現(xiàn),早干預(yù),有效促進(jìn)嬰兒語言發(fā)育。

Clinical significance of neonatal hearing screening in the diagnosis of hearing diseases

[Abstract] Objective To explore result analysis and clinical significance of neonatal rapid auditory brainstem evoked potential response in hearing screening. Methods 3000 neonatal clinical data for hearing screening were collected and analyzed in our hospital from January 2014 to January 2015.Rapid and noninvasive auditory brainstem evoked potential response detection was used in 3000 neonates by hearing screening instrument of auditory brainstem evoked potential response in order to get known the morbidity of neonatal hearing disorder. Results Among these live neonates,472 babies were failed in preliminary screening,and positive rate was 15.7%(472/3000).Forty-two days after birth,464 out of 472 babies were screened again,and 442 neonates were passed.The passing rate in secondary screening was 95.3%. The rest were failed accounting for 4.7%.There were 20 cases transferred to hearing screening center in our city and they were finally diagnosed as hearing disorders and provided with intervention treatment. Seven cases diagnosed as binaural sensory deafness were classified into moderate hearing loss,and took 0.23% among all participants.The auditory brainstem response examinations were all normal in 4 months old in 3 cases, 6 months old in 4 cases,and one year old by follow up in 8 cases.Indexes of different delivery mode,gestational age,and birth weight were closely related with hearing damage. Conclusion By auditory brainstem evoked potential response for hearing screening,neonatal hearing loss can be detected and intervened at an early stage, which effectively promotes infant′s language development.

聽力障礙是常見的出生缺陷,在我國屬5項(xiàng)殘疾之首,本省新生兒聽力障礙的發(fā)生率為1.02‰左右[1]。新生兒聽力篩查可早發(fā)現(xiàn)、早干預(yù)聽力障礙兒童,對嬰幼兒的言語發(fā)育等起至關(guān)重要的作用,分析新生兒先天聽力障礙發(fā)生的相關(guān)因素,相應(yīng)采取預(yù)防措施,減少新生兒先天聽力障礙發(fā)病[2]。聽覺腦干誘發(fā)電位(brainstem auditory evoked potentials,BAEP)通過對發(fā)射純音描記耳蝸、聽神經(jīng)以及中樞系統(tǒng)對純音刺激反應(yīng)的測定方法,具有客觀性強(qiáng)、檢測結(jié)果穩(wěn)定等優(yōu)點(diǎn),F(xiàn)將本院新生兒快速腦干誘發(fā)電位聽力篩查資料進(jìn)行分析。 (黃鉆娣 凌琴音 周軼)
